Cancel” button not working on “Suggest a video” form (TV inner pages)

On WPFolks TV inner pages (e.g. https://wpfolks.org/tv/dqbzmjvqi6m/), when using the “Suggest a video” form under “What to watch next,” clicking the “Cancel” button does nothing — the form stays open with the entered content still present instead of closing/resetting.

Steps to reproduce:

1. Go to a WPFolks TV inner page, e.g. https://wpfolks.org/tv/dqbzmjvqi6m/

2. Click “Suggest a video”

3. Enter a search term (e.g. in the video search field) and/or type text in the reason field

4. Click “Cancel”

5. Observe nothing happens — the form does not close or reset

Expected: Clicking “Cancel” should close the “Suggest a video” form and discard/reset any entered input.
