Jadav Form Lead Qualification is a lightweight form builder for creating responsive forms and managing submissions inside WordPress.

Jadav Form Lead Qualification focuses on first-party lead qualification. It stores campaign attribution with each entry and calculates a local lead score from submitted intent signals, logged-in user context, uploaded files, and UTM data. The scoring runs on the site and does not call an external API.
Privacy
Submissions are stored in your WordPress database. If enabled per form, IP address, user agent, UTM values, and referrer URL are stored with the entry. The lead score is calculated locally on your site.
